First make sure you have a copy of your standard format default document
template saved along with all your other templates. 
 
In my Writer, I see four tabs (Documents, Spreadsheets,
Presentations
and Drawings), and under Documents there are two widgets - one marked
"My Templates" and one marked MediaWiki.
 
Go into "My Templates" and select the template you would like to use as
default. After you select it, there will be a tab up top that says "set
as default".

This does not work.

The File -> New menu has a "Tamplates" option, but all that does is go
to a window where you can select a specific template that, as far as I
can see, consists only of templates I have already created.
