Hi there,

I am quite confused... I have been spending all day trying to understand how
to create an alphabetical index for an ebook I am working on.
I have read this:

but I am still at a loss... 
For example I would alike to get the page numbers on all entries (like in
the doc I linked above), not just the secondary ones. Also I would like to
have entries that don't have secondary entries... at the moment I can only
get a primary entry and a secondary entry that is identical to the primary one.
Not to mention making the index made of hyperlinks (the table of contents is
not an issue), just like, you guessed it, the file above.

Not sure if I am making any sense here... Perhaps you can direct me to some
docs where things are described a bit more in depth.
