Here are the steps I took. Tools, Customize, Menus. I added a new menu
(call it Billing), then under menu content I clicked add, application, new
document from template. This adds a menu item called Billing. Tools,
Options, LibreOffice, paths would point to the template directory on the
network. In LO3.5 this would open the template the user chose, then prompt
them for input fields. In LO4.0 it opens the template manager, and they can
still open the desired template, but it doesn't prompt them for input.

Your way of adding an icon performs the same way as adding the menu item.
However, it still won't prompt the user for input.
Unless someone has another idea, we will create an application that will
open the template from the network location. It's an added step, and takes
almost 1 minute to open the template, even if LO is already open. I've
increased the memory on the Graphics cache to 256 MB, but we aren't dealing
with graphics, so that didn't appear to make any difference (just trying
different things to speed it up). Any other ideas?

However, if I click the /Toolbars/ tab with the Customize dialog
open, I see the Toolbar page. /Standard/ is the default value for the
dropdown menu of toolbars. There I see /New Documents from Template/. If
it is not checked, I can check it. This places its icon in the Standard
toolbar directly below the Menu tool bar approximately below /Edit/. Now
when I want to use one of my templates, I click the icon, the Template
Manager opens. I then click /My Templates/ and select the template I
want to use.
I describe what I have done as:
Tools -> Customize -> Toolbars. Check /New Documents from Template/ if
it is not checked. Click OK to close the dialog. Now the Standard
toolbar has the /New Documents from Template/ icon. Any time you have
Writer open, you click this icon to open the Templates Manager. From
here you can select the template you want to use.

I created numerous templates in LO 3.5.5. I created the letter/template
and inserted input fields that would stop and ask the user to input the
appropriate data. Worked real well in LO 3.5.5.
We are hoping to upgrade to version 4.0.0.3, but when we try to copy or
import the templates, they don't stop at the input fields to prompt the
user to input the data.
We have tried to retype the letters/templates, inserting input fields,
to no avail. Is there a way to get the template to stop and prompt the
user for
each input box?
